Health History

Camper health forms should be submitted a month prior to your camper’s arrival to meet Colorado’s state licensing regulations. Campers cannot attend without meeting these requirements for their safety.

Required Health Forms

General Health Appraisal from a physician, physician assistant or nurse practitioner from the last 12 months.

Immunization Records:

Campers who are Colorado residents are required to upload their immunization history using the Colorado Certificate of Immunization. Families can request and generate this certificate through the Colorado Immunization Information System (CIIS) Public Portal at https://copublicportal.state.co.us/

Campers who reside out of state or outside of the United States may submit their immunization history using any official form provided by a healthcare provider or country of origin.

Additional Health Forms

Medication Release Form (required for any routine prescription, over-the-counter medications or supplements) signed by a healthcare provider in the last 12 months. One form per medication, completed in full with medication name, dosage, route, time and frequency. A parent or guardian signature must also be on this form for our health team to administer these medications.

Emergency Anaphylaxis and Asthma Plan (required for campers with allergies and/or asthma who are at risk for anaphylaxis and bringing lifesaving medications to camp).

Bicycle Inspection

All campers who are bringing their own bikes to camp, must have a bicycle inspection form completed by a bicycle mechanic.

Financial Assistance

The Y believes all children deserve an overnight camp experience. That's why we provide scholarships to hundreds of campers each summer. For information on YMCA Camp Tumbleson Lake's financial assistance program, email our camp director. 

Changes & Cancelations

A $300 deposit is required for all programs.

If a cancellation is needed before May 1st, a refund or credit can be issued to the customer. The YMCA will retain the $300 deposit.

If a cancellation is needed after May 1st, no refund may be issued, but a YMCA account credit can be applied depending on cancellation reason. This credit is valid for one calendar year. The YMCA will retain the $300 deposit.

Exceptions to these policies can only be made by the YMCA Executive Director of Resident Camp. Exception requests may be granted when extenuating circumstances arise.

Camp Store

We have t-shirts, sweatshirts, bracelets, stickers and more, ranging in price from $3 to $50. You can put money in your camper’s account before they arrive, so they can buy goodies throughout the week.
